When you meet with our Care Coordinator — here's what you can expect:
Our Care Coordinator will ask questions to discover where your loved one could benefit from a helping hand, along with which aspects of daily life are currently going well.
ext, we will ask about common Activities of Daily Living — bathing, dressing, cooking, transferring in and out of a chair or bed. Are they still manageable, or is some assistance needed?
Once we have assessed the situation, we will explore care options your family may benefit from, based on any gaps or needs identified during the consultation.
A review of the home environment will help identify any safety concerns — such as trip hazards, lack of grab bars in wet areas, or other risks that could affect your loved one's safety at home.
Our Care Coordinator will also identify any physical or emotional risks associated with social isolation and loneliness, which can significantly impact overall health and wellbeing.
Finally, we will discuss any health concerns, medication management, and other issues that should be addressed in a personalized care coordination plan tailored to your loved one's needs.
